I am about going to show you how you can insert a form on your website. I want  to assume you want a form that must have the following fields: Name, E-mail address, Sex, and Tell us your views about our website. Now I am now going to walk you through the process of creating just this. I will be using Microsoft Front Page.

1. Place insertion point on the area of your web page where you are going to insert the form. Go to “Insert”, then “form” and then “form”. This action places the form box on your web page. Type the title of the first field (Name) just behind the “submit” button.

2. Now go to “Insert”, “Form”, and then click on “Textbox”. This places the Name field box into your web page. Press “Enter” on your keyboard (the “Submit” and “Reset” button moves to the next line”. Type E-mail Address and go to “Insert”, “Form” and click again “Text Box”. The “E-mail field box is also inserted. You can do this for every field that must have a “Text Box”.

3. The “Sex” field must however have what is called an “Option Button”. So this is exactly what you will click on for this field instead of “Textbox”. You will type “Sex”, then “Male” and then insert one option button, then type “Female”, and then insert the second option button.

4.  You will need a different type of form field (Text Area) for the last field name “Tell us your views about our school”. You can follow the same process above but in this case you will have to click on “Text Area” to insert the “Text Area” field box. You can then finally press “Enter” for the “Submit” and “Reset” buttons to move to the next line.

5.  You now need to define the field properties for each field box. You must do this for the fields to work properly. I am starting with the “Name” Field. Click on the box and then right click on it and in the drop down menu, click on “Form Field Properties”.

6.  A “Text Box” properties box appears. In the Name box, type a name to identify the text box. This internal name, which is not shown on the form, identifies the text box in the form results. It will be visible to the site visitor on the default confirmation page, which is displayed when you use a form handler rather than a custom script. If you want text to appear in the text box when a site visitor first opens the form, type the text in the Initial value box. In the Width in characters box, type a number indicating how many characters wide you want the box to be. In the Password field, click Yes or No to determine whether or not the field contains a password so its contents can be encrypted. Click the Style button to access other options for formatting your form using cascading style sheets as an inline style. If you click the “Validate button”, the “Text Box Validation box” will show up. This box allows you to include more properties for this form field. You can click okay after you must have defined the properties. NB: You will have to carry out this process for all the other “Text Box” fields in your form.

7.  You are now going to look at the options you have for the “Option Button” properties box. Double-click the option button or click on it and then right click on it. In the Group name box, type a name to identify the group to which this option button belongs. This internal name, which is not shown on the form, identifies the option button group in the form results. It will be visible to the site visitor on the default confirmation page, which is displayed when you use a form handler rather than a custom script. In the Value box, type the value to associate with this field. If this option button is selected, this value is returned with the form results, and is displayed on the default confirmation page. Click Selected under Initial state if you want the option button to be selected by default when a site visitor first opens the form. Click the Style button to access other options for formatting your form using cascading style sheets as an inline style. You can also “validate” this box just like we did under “Text Box” properties.

8.  You are to carry out the same step when you define the “Text Area” properties. You can do this now and see what you will get.

9.  It is now time for you to do the same thing for the “Submit” and “Reset” button. Click on it and then right click on it. Click on form field properties as usual. The “Push Button Properties” Dialog Box appears. In the Name box, type a name to identify the button in the form results when you receive them. This internal name, which is not shown on the form, identifies the push button in the form results. In the Value/label box, type the text that will appear on the button. In the Button type box, select “Submit” if it is not already selected. Click the Style button to access other options for formatting your form using cascading style sheets as an inline style.

10.  You should also do the same for the “Reset” button

11.   When you must have carried out this process for all the fields, you are now ready to define the form properties for the whole form. You can click on the “Submit” button (you can actually use any of the form fields to do this), then right click on it and then click on  “Form Properties”

12.  In the “Form Properties” Box that appears, select the “Send to File name check box if it is not already selected and copy the e-mail address where you want the form result to be sent to in the “Email address” box (if you want the form results sent to an e-mail address). You can leave the file name as it is in the “File name” field. You should also enter the form name in the “Form name” box.  You should only select the “Send to other” check box if you want your form result sent to a database and not an e-mail address. You can leave the “target frame as default. Click Okay when you are through. You don’t have to define “Form Properties for all the fields as done when you defined “Form field properties”. When you do this for one of the form fields, it appears for all the other fields.

You have created your first form and I must let you know that for your form to work properly, you must publish your site to a server that has the “Microsoft FrontPage extension” – Your hosting company will tell you more about this.
